Step 2: Admission’s Office

Please complete and send in the following:

  1. Medical Release (online version)
  2. Health Appraisal (print and submit)
  3. Immunization Record. (Send photocopy from family doctor)
IMMUNIZATIONS:  Michigan State law requires our school to be in compliance with the current immunizations required for attendance. These requirements are subject to change. Your local health department can give you the answers you seek as well as your family provider. These immunizations must be done BEFORE the first day of school. The form that the State of Michigan requires us to have on campus is the MCIR (Michigan Care Improvement Registry) that can only come from your health provider or local health department. It MUST be signed by your provider or the assigned contact at the health department. If you are not a MI resident, please provide GLAA with a printed list of immunizations BEFORE the first day of school. Please be sure your student is up-t0-date.
WAIVERS:  Some families choose to waive immunizations, but they must meet Michigan State law for our school to be compliant. If there is no history of immunizations given and you are a MI resident, you MUST make an appointment at your local health department to get a waiver for immunizations refused. This must be done BEFORE the first day of school. Some families may follow their own schedule and receive some and refuse others. A MCIR form is required for those received and a waiver for those refused. If you are not a MI resident, a waiver is still required BEFORE the first day of school.
  4. Copy of Health Insurance Card. (Send photocopy of BOTH sides of card)
  5. Computer Use Agreement
  6. Campus Leave Authorization (online version) OR  Campus Leave Authorization (printable version)
  7. Consent to Release Academic Records (online version) OR  Consent to Release Academic Records (printable version) requesting the CA60 Folder (cumulative folder) and transcript from last school attended.

COVID-19 Update

On April 2, Michigan Governor Gretchen Whitmer rolled out Executive Order 2020-35, which mandates a “suspension of in-person K-12 instruction for the remainder of 2019-2020 school year”

Unless restrictions on public gatherings and use of school buildings is lifted before the end of the 2019-2020 school year,” it is unlikely that we will have graduation this year as scheduled on May 24, 2020. Unless the pandemic miraculously disappears sooner than expected, Great Lakes Adventist Academy will seek an alternate date in the future to hold a graduation ceremony for our seniors.

Great Lakes Adventist Academy, along with the other schools in the Michigan Conference, will continue forward with distance education for the remainder of the 2019-2020 school year.

In an effort to keep our school family safe during this time, and to remain in compliance with social distancing orders and recommendations, we are kindly asking that you be patient and wait until those restrictions are lifted to come and get your child’s belongings and move them out of the dorms.

Parents and students will be notified via email of any updates.
