International Student Application Process

Welcome to the GLAA application process check sheet.

We are happy you've chosen to become part of our school family. To be eligible for enrollment and employment at Great Lakes Adventist Academy please complete the following items in the steps below.

**IMPORTANT: Steps 1 and 2 must be completed before an I-20 will be sent.

 

Step 1: Registrar's Office

a. Download and fill out application for upcoming school year.
New Student Application

Returning Student Application
Tip: Make sure you and your parent/guardian sign application. Make sure it is filled out completely. Send it in early!

b. Download and distribute three Recommendation Forms.
Recommendation Form
Tip: Provide a self-addressed-stamped envelope for the convenience of your references or urge them to fax the form ASAP. One should be an Administrator/teacher from the last school you attended.

c. Submit application fee of $20.

d. Submit U.S.A. guardian information (name, address, phone number).

Your application will be taken to the Admissions Committee at this point and you will receive a letter of acceptance.

Step 2: Registrar's Office
Please complete and send in the following:

a. Medical Release. Medical Release Form
b. Health Appraisal. Health Appraisal Form
c. Immunization Record. (Send photocopy from family doctor)
d. Copy of Health Insurance Card. (Send photocopy of BOTH sides of card)
e. Computer Use Agreement. Computer Use Agreement Form
f. Campus Leave Authorization. Campus Leave Authorization Form
g. Consent to Release Student Records. Transcript Release Form

Note: Application will be taken to Admissions Committee after Steps 1 and 2 are completed. An I-20 and a letter of acceptance will then be sent.

Step 3: Business Office
Please complete and send in the following:

a. I-9 I-9 Employment Eligibility Verification Form
b. MI W-4 Michigan W-4 Form
c. Federal W-4 Federal W-4 Form
d. Tithe Agreement Tithe Agreement Form
e. Birth Certificate (State Certified)
f. Social Security Card (Students may apply for a Social Security card during the 2nd month of attendance.)

Step 4: Business Office

a. Payment in full must be received. All fees must be paid in United States dollars. Please mark your check "U.S. Funds." All students that are not from the United States or Canada must pay their total minimum charges for the year in advance less the cash discount.
The amount that must be paid is $14,350 U.S. funds. This amount will be due on the day that the student enters school.

Registration Options:

Early Registration By Appointment. Steps 1-4 must be completed before an appointment can be made. Please call (989) 427-5181 to schedule an appointment.

Open Registration on Campus (No appointment needed). Bring all items not submitted from Steps 3-4.

*Refer to Calendar of Events for specific dates for early and open registration.
